Step 2: Email the completed forms back to cisa@common-insurance.com along with your updated AHIP, DOI health license, and your E&O.

Step 3: Clever Care will email you instructions on how to certify online through their broker portal.

Step 4: Once you have completed the certification you will be notified by Clever Care of your ready to sell status.
